New Heart, Inc. (“New Heart,” “we,” “us,” or “our”) respects your privacy. This Policy applies only to information collected through the Site. It does not apply to information collected offline or through our clinical services, patient portal, or electronic health record (EHR). If you are a patient, information we create or receive about you in the course of providing healthcare services is Protected Health Information (“PHI”) and is governed by our HIPAA Notice of Privacy Practices, not this Policy. Please do not include sensitive medical details in open website forms. For patient communications involving PHI, please use our patient portal or call us.
